The Position

Are you an experienced WMO consultant with extensive knowledge of support within the WMO? Can you analyze complex support needs well, implement the right assistance, and independently make suitable assessments? Then we are looking for you!

For the Utrecht region, we are looking for two experienced WMO Support Consultants to temporarily strengthen our team.

What will you do?

As a WMO Support Consultant, you will investigate the support needs of residents who require help to function as independently as possible.

You are an enthusiastic and committed professional with a heart for people. You have experience as a WMO consultant and are familiar with the challenges and opportunities within the Social Domain. You are strong in communication, enjoy teamwork, and are capable of making independent decisions. Furthermore, you are flexible and willing to work for different municipalities.

Offer

Working through YER offers security combined with variety and challenge. This includes the following employment conditions:

• Conducting kitchen table conversations and analyzing support requests.
• Mapping the personal situation, limitations, and capabilities of residents.
• Assessing applications for support from the WMO.
• Drafting suitable support plans and assessments.
• Reviewing complex cases and making careful considerations.
• Maintaining contact with residents, care providers, and other involved parties.
• Completed relevant HBO (higher professional education) degree, such as Social Work and Services (MWD), Social Pedagogical Assistance (SPH), Social Work, or equivalent;
• Minimum of 3 years of work experience as a WMO consultant within a municipal context;
• Experience with complex case management support
• Current knowledge of WMO legislation and related regulations;
• Strong communication skills and empathy;
• Strong plus: Experience with sheltered living and Zorgned
• Competitive salary depending on knowledge and experience, in accordance with scale 9 of the Municipalities CAO;
• Also possible on a freelance basis
• Fixed-term or permanent contract.
