- Support for patients and relatives in organizing post-hospital care through concrete measures and assistance
- Differentiated social-legal advice and information for patients and their relatives
- Initiation and organization of concrete measures and assistance, such as support for nursing care needs, home care, medical and social rehabilitation, as well as initiation of legal guardianship
- Regular coordination and cooperation within the interdisciplinary team
