LeistungsbeschreibungGemeente Stichtse Vecht is gradually building a new municipal access point for Youth Care and Wmo Guidance from May 2026 onwards. For this, we are looking for experienced consultants who want to contribute to the development of a future-proof access point and at the same time support residents with appropriate help and support.You will join a team under development. In addition to carrying out substantive activities, you will make an active contribution to the development of processes, working methods and cooperation agreements.

We deliberately start on a small scale in order to gain experience, learn and carefully shape the service provision.

The complexity of the issues requires professional judgement, an integral perspective and good cooperation with chain partners.Responsibilities:Conducting conversations with residents about support questions concerning self-reliance and participation.Investigating the support request and assessing the necessity for Wmo guidance.Assessing and issuing decisions for customized provisions within the Wmo.Stimulating self-reliance, use of the network and general facilities where possible.Directing the provided support and monitoring results.Cooperating with care providers, network partners and other involved professionals.Identifying changes in situations and adjusting in time where necessary.Requirements:Completed HBO education, for example Social Work, SPH, MWD, Occupational Therapy or Nursing.Demonstrable experience as a Wmo consultant.Knowledge of the Wmo, indication processes and relevant legislation and regulations.Requirement:Experience in conducting kitchen table and/or family conversations.Strong analytical skills and the ability to make balanced decisions independently.Good written skills for reports and decisions.Cooperative and able to create connections with residents, networks and chain partners.Experience within a municipal access point or social domain is an advantage.Preferred Qualifications:Independently deployable as a Youth and/or Wmo Guidance consultant.Able to become productive quickly and independently manage a caseload.Experience with assessment, reporting, issuing decisions and case direction.Flexible and able to work well within an organization that is fully under development.Able to deal with changing work processes and new cooperation agreements.Über den AuftraggeberStart: 19.10.2026End: 19.04.2027, with an option to extendWorkload: approx.
